Position: Nurse Assessor - Home based - Full time or part time permanent. Location: Home-based - covering a designated region Vacancy: Permanent - Full Time Salary: £34,000 - £38,000 plus Car allowance available (negotiable depending on location) Hours: 37.5 Gray Healthcare is a specialist health and social care provider, supporting people with mental health needs, disabilities and long-term conditions looking to step back into the community from long-term in-patient support services. We have some fantastic opportunities for specialist nurse assessors a to join us at this critical point in our growth strategy. We are looking recovery focused registered nurses (RNLD learning disabilities or RMN mental health) to join our team as nurse assessors team across the country. Our nurse assessors work from home, and are required to travel to complete assessments within their geography on either a full time or part time basis. To be considered, we would like to see experience in: Working with people with mental health, complex needs, ABI or learning disabilities (active pin required) Experience or understanding of CHC or transition of individuals moving from hospital/residential settings into the community Ability to work collaboratively with community partners to promote integrated care pathways Undertaking assessments and writing funding reports for placements Experience or understanding of commissioning processes You will also need evidence: Excellent assessment skills, including risk assessment and risk management planning Solid presentation skills and confidence to work with a wide range of partner agencies Excellent communication skills; including strong report writing skills and computer literacy Ability to work under pressure Willingness to work across geographical areas Innovative thinking that supports bespoke solutions for people returning to community based supported living For more information about the role, or to find out about Gray Healthcare and our recruitment processes, please contact our recruitment manager Richard.carrington @grayhealthcare.com Job Type: Full-time Salary: £34,000.00 to £38,000.00 per annum depending on experience (pro-rata is part time)
